I have no history of skin cancer

I have not used Accutane (Isotretinoin) in the last 6 months

I am not taking photosensitising medications (e.g. tetracyclines)

I am not on anticoagulants (e.g. blood thinners)

I have not had gold therapy (risk of blue-grey discolouration)

I have no medical implants in the treatment area

I do not have a pacemaker

I do not have vitiligo

I do not have active herpes simplex (cold sores) in the treatment area

I do not have diabetes or other conditions affecting wound healing

I have no open wounds or skin infections in the area to be treated

I have no history of keloid or hypertrophic scarring

I have no history of thromboembolism, hypercoagulability, systemic disease,

connective tissue disease, or immunosuppression

I do not have dysplastic nevi (abnormal moles) or suspicious pigmented lesions

I have avoided sun exposure, tanning, or self-tan for at least 2–4 weeks

I have disclosed all current medications and relevant medical history
